Good morning everyone

Document says (Assessment) DM patient presents with hypoglycemic symptoms.
Can I code 250.8X or I should could 250.0X

As per the document there is no confirm diagnos "hypoglycemia" . But in the history part he document that patient had dizziness after he took the DM medicine.
