Where can I get the guidelines to billing for the Diabetes Education Billing for a provider?

We employ Diabetic Educators and bill incident to the provider.

Iowa Medicaid has a good instruction on their web site. Its titled Nutrition Therapy provided by non-physician staff.

We use G0108 or G0109. You could probably find information on the CMS web site too.

How do you bill them without the patient being charged a copay every time you bill out the G codes for the sessions? Can the G codes be billed out for the entire 10 hours all at once?
