If the physician lists diabetes, and diabeties with ketoacidosis, should I code both or just 250.10 for the Ketoacidosis?

Just the 250.10 Diabetes with Ketoacidosis. Coding diabetes and the diabetes with ketoacidosis would be redundant. The 250.10 covers everything. Hope that helps

Just 250.10 is enough as it includes both diabetes as well as diabetic ketoacidosis

Diabetes Mellitus With Ketoacidosis, Type Two or unspecified type, Not Stated As Uncontrolled; Fifth-digit 0 is for use with type two patients, even if the patient requires insulin
Diabetic: {acidosis} {Ketosis} without mention of coma
