I have read many topics on this ; one say to code diabetic chronic renal disease when both are documented and the other say to code diabetic nephropathy when doctor has documented. Which is true?

There is not an excludes note for these two codes. Additionally, if the staging of the CKD is documented, be sure to include that as well.
