I recently started working in a Podiatry office and the diabetic shoes are a mess. Can anyone tell me if they have set procedures in place for obtaining documentation for diabetic shoes? Or if you have any forms you use to send to the PCP? I am having a lot of trouble getting the documentation I need from the PCP for the diabetes note and foot eval. Their notes will barely mention diabetes, if at all, and they will never have anything about the qualifying foot conditions.
