I understand that the documentation must support the diagnosis, but my question is, who can legally change a diagnosis code? For example, if a provider uses an unspecified code, but the documentation supports something that is more specific. Who has the ability to change the code? The provider? A CPC?

Any help would be appreciated. Thank you
