I am looking for the dx code for Nodular Hyperplasia with Oncocytic Metaplasia. The only thing I can come up with is 246.8.
 
The patient had a right thyroid lobectomy the path findings are below:

---Final Pathologic Diagnosis---

A. Right thyroid lobe, excision:
d Nodular Hyperplasia with Oncocytic Metaplasia.
d Chronic Lymphocytic Thyroiditis, Focal.

You are overthinking this, it's nodular hyperplasia, aka, thyroid nodule, with Oncocytic Metaplasia, HurthleCell, but no mention of carcinoma, therefore
ICD9 226 - Neoplasm Thyroid Benign......
