It is not possible to address your question without first knowing what you are wanting to code. What was documented by the provider.
Patient was only seen in the clinic (prof fee coding) for a follow-up visit. The provider listed these two diagnosis codes in his assessment. He is currently treating both conditions.

It is still not clear what conditions exactly the provider documented and were both undergoing active treatment.
The physician is seeing and actively treating the patient for Malignant neoplasm of colon and iron deficiency anemia due to chronic blood loss. In his assessment he listed the colon cancer first and also addressed the colon cancer first in his HPI as the primary reason for the visit.
