I am looking for a good training session, book, webinar, etc. to help educate coders on the proper procedure for diagnosis coding. I am trying to clarify certain items such as not billing a diagnosis and the related symptoms, if it is ok to code from things stated in the HPI and ROS, what related documents in the chart can be used for obtaining a diagnosis, etc. This is specifically for physical office coding. Any suggestions would be appreciated!
