DX:
Physician has given NYHA Class III heart failure with LVEF less than or equal to 35%. Can someone please help me find the diagnosis code for this?

Is QRS > 120? If this is for primary prevention then code what is known, heart failure. Primary prevention has its own rules. You will need to bill with modifier Q0 and add the NCT # and possibly Z06.0 see Medicare chapter 32 270.2 and IDEs and NCD 20.4

Yes this is for primary prevention. Where do you get a NCT#?
