Hello,
Looking for input on diagnosis coding for Cancer Surveillance.

Right parotid low/intermediate grade mucoepidermoid carcinoma: Patient doing well with no evidence of disease recurrence at 6 years out. Recurrence of disease is exceedingly rare at this time, recommend annual follow up
- Continue surveillance in survivorship Clinic, patient will return to clinic if notices anything concerning.

Would I only code for diagnosis

or would I code it
