Trying to make sure I am sequencing my diagnosis correctly, getting confused on some of the articles im reading and I want to make sure im understanding them, anyone would like to help, making a little quick cheat sheet for myself




First screening colonoscopy w/or without findings = Z12.11, findings secondary (if any)
No personal or family history of colon polyps or cancer


First screening colonoscopy w/or without findings = Z12.11, Z83.71, findings secondary (if any)
No personal hx, BUT FAMILY HX of colon polyps


Second (subsequent) diagnostic/screening colonoscopy = indication to why a colonoscopy
No personal/family hx of polyps/ No findings found in colonoscopy (normal)


Second (subsequent) screening colonoscopy = Z86.010, findings secondary (if any)
Personal hx of polyps, no family hx


Second (subsequent)screening colonoscopy = Z83.71, findings secondary (if any)
No personal but Family hx polyps

How to code surveillance colonoscopy AFTER adenomatous polyps have been found in the screening colonoscopy. I need help with DX and correct modifiers if any need to be appended.
