Wiki Diagnosis coding

Four of our claims were rejected because according to the insurance company we do not have diagnosis that supports medical necessity. I don?t understand ... Each patient has a sensorineural bilateral hearing loss and diagnostic code 389.18 was used. And we can?t change a patient?s diagnosis. Please help

The procedures are 92590, V5298 , V5014 (2), V5014 and the insurance companies are Anthem and Medicare. After doing more research the Medicare patient shouldn?t' have been billed.
