Wiki Diagnosis for billing G2211 (Longitudinal care)

Can G2211 be billed with any diagnosis (for example, a wellness code)? Or does it have to be billed with a problem diagnosis or specifically a chronic condition diagnosis code? If anyone has had denials, I would appreciate hearing of this.

As G2211 is not billable with 9938x-9939x, I would not use it with a wellness diagnosis.
