Diagnosis found on PMH and they are on current medications for them

Hi,

I am looking for some clarification if anyone could add there input.

If patient's have in there H/P diagnosis of Gout, A-Fib.... and they are on current medications for them at our Facility because they are on meds. for them we pick them up. My question comes do I could them as chronic conditions because they are on long term meds?

My example is Gout... patient is on Zyloprim... do I just code as Gout, unspec M109. or do I code as Gout, chronic M1A9XXO.:confused:

I would just code it as unspecified gout M10.9 the provider would have to document that it's chronic gout in the record.
