In an outpatient setting, does the Medicare Part B billing physician still bill 90935 for non-ESRD patients (acute kidney failure)? And is it a 90935 for the entire month or just per dialysis treatment?

Dialysis Billing Non-ESRD patients, acute kidney failure

I learned today that we are to bill differently for acute kidney failure, non-ESRD related outpt billing. The way I understand it is, bill 90935 or 90945 per treatment and make sure the setting is not inpatient hospital. Also, I'm assuming the dx code would be acute renal failure and not ESRD or a CKD code. Have you received information on this from CMS or a newsletter from an ins co?
