You might could bill for the procedure but it depends on what the note states, however since it was completed with irrigation by RN then you could bill only for the irrigation, not both.
Procedures that are started and go thru all the necessary steps but do not accomplish the goal can be coded and charged. If the procedure was started and all the steps not completed, it can be coded with a modifier, however when a different procedure is also performed for the same purpose, you cannot code for both.
