The doctor I work for is constantly having to spend a lot of time answering disability requests for patients. Is there a code that can be used so he gets paid for his time?
 
It is the patients responsibility to pay these. We charge 25.00 for them and the patient doesn't get them until they pay. I hope that helps.
 
If your provider meets the criteria described in the 9945X CPT codes he/she can report those codes which include completion of necessary documentation/certificates and report. In order to be able to answer all of the questions asked on these forms my guess would be that he/she has provided these services. Take a look at these codes and discuss with your provider... the patient may have to pay, but if the form is requested you can bill to the person that requested the information.
