Wiki discharge from IP - admit to SNF on same date

Can anyone tell me if a doctor discharges a patient from IP to SNF on the same date.....is the doctor able to chart the admit from the IP setting or does the admit require a face to face by that doctor on the site of the SNF (face to face)
 
The H & P codes for SNF state time spent at patients bedside and on the patients facility floor or unit. Our physicians round twice a day. The morning round is usually when the decision made to disc from IP and admit to SNF so they will do their face to face IP discharge with the patient. When they do their evening rounds they will see the patient again in SNF and do their SNF H&P. Hope this helps.
