If a patient is discharged and then admitted again the very next day for the same diagnosis would that discharge turn into a subsequent note and it just continue on until discharge again?
Or would you just bill a discharge and then another H&P when the patient is admitted again?

I assume that you are coding for the admitting physician.
If the admitting physician did the discharge that day, I would code for the discharge. If the same physician had to admit the patient the very next day, and had to do another H&P (which is most likely), I would code the admit for that DOS using the appropriate level of service.
