We had a patient come in for a HPV vaccine but he was uncooperative and the vaccination didn't occur but an encounter was still generated. The nurse coded it using 99211 but the patient has never been seen at the clinic, can I add modifier 52 or do we use a new patient office visit even though he just saw the nurse? Please advise.

No there is nothing you can code for this. This is a new patient and the nurse cannot be the only person to evaluate the patient, also you cannot use a 52 modifier with an office visit.
