Hi. I have a coding question on how to accurately apply E11.69 and E11.59. In reviewing charts I had listed in both past medical history list and problem list, the diagnoses diabetes associated with hypertension (coded E11.59 and I10) and diabetes associated with hyperlipidemia (coded E11.69 and E78.5). In the body of the note the provider did not discuss the HTN, HDL or either diagnosis being associated with their diabetes. Using the MEAT criteria, both E11.69 and E11.59 are not causal codes which require the provider to specifically link these diagnoses to the DM. Would you have picked up with E11.69 and E11.59 only from PMH and PL and captured them respectively? Thanks for any insight you can provide me.
