can 250.00 be coded with 356.9? or should it be coded as 250.60 and 357.2?our encounter forms do not list anything except 250.00(1,2,3). so our docs check the DM w/o mention of complications; then they check 356.9 neuropathy, peripheral unspec. what is the proper coding? is documentation needed to use 250.60/357.2? Thanks:confused:
 
The doctor has to specify that it is "diabetic neuropathy" or the neuropathy is "due to" I would query the provider. If it isn't linked in the chart note by similar statements then the 250.00 and 356.9 respectively are the most correct. We cannot assume the linkage
