"Hi all, can we code DM and dermatitis together as a combination code even if the provider did not mention that the dermatitis is due to DM?"

Thank you In Advance

See ICD-10-CM 2025 guidelines for "With", these conditions (diabetes and dermatitis) should be coded as related even in the absence of provider documentation explicitly linking them, unless the documentation clearly states the conditions are unrelated... This classification presumes a causal relationship between the two conditions linked by these terms in the Alphabetic Index or Tabular List. See the ICD-10-CM Alphabetic Index: Diabetes>with>dermatitis E11.620

There are many conditions that have a causal relationship "with" diabetes; the exception is NEC codes. Coding Clinic has stated that the "with" guideline does not apply to NEC index entries that cover a wide category or condition. IE: Diabetes>with>arthropathy NEC, or ...with>renal complication NEC, or ...with>skin ulcer NEC- the provider has to link these to diabetes to capture the combo code.

Thank you so much for the informative lesson today! I haven't had an ICD 10 book in a couple of years. My job provided
HCC coder and realized I missed the guidelines. I just bought a book. Again appreciate your help!

Thank you so much for explaining it so well!!

No. The only causal relationships we can assume correlations are: CKD,PVD, cataracts and neuropathy.
That is simply an incorrect statement. Under the ICD-10 'with' and 'in' rules, there is an assumed causal relationship with any condition listed under 'with' and there is no need for provider linkage to code any condition listed after the 'with' in the index UNLESS provider has linked the condition to another cause , OR the condition is an NEC code. So in the ICD index you see Diabetes>with>dermatitis. You will auto link and code the combo unless provider has linked the dermatitis to another cause. I am not sure where you got the idea that CKD, PVD , cataracts and neuropathy were the only auto links. Certain projects do have certain guidelines, so perhaps thats it. But under ICD-10 guidelines you will auto link conditions that follow 'with' or 'in' without provider linkage unless provider links to another condition or its an nec code such as 'circulartory complication nec' or skin complication nec etc.

Yes absolutely you CAN. Under ICD-10 'with' and 'in' rules there is an assumed causal relationship with any condition that follows with or in and can be linked without provider linkage, as long as provider has not linked the condition to another cause. In your ICD-10 book index Diabetes>with>dermatitis and code it. You only will not if provider has linked the condition to another cause or it is an NEC code such as 'circulatory complication nec' or 'skin complication nec' etc. all others that are not nec, auto link and code the combos.
