I am unsure if there is a causal relationship here. Does "uncontrolled" WITH foot ulcer establish the causal relationship? Or does "diabetic" foot ulcer have to be documented in order to establish that causal relationship?

That is incorrect.

Per Coding Guidelines: Section 1, A, 8, it states

The word "with" should be interpeted to mean "associated with" or "due to" when it appears in a code title.

So in this situation, DM 1, uncontrolled with foot ulcer would mean the same as diabetic foot ulcer.

Even though the coding guidelines indicate the above, there is a coding clinic from 2009 indicating that Diabetes with neuropathy should be coded as a causal relationship.

Thus the reason for my question for the DM 1, uncontrolled with foot ulcer.
