We are billing for Optometry items to our DME carrier and they are being denied for the rendering providers name and PTAN. I cannot figure out where this goes on the 1500 claim form. We have our billing provider info in box 33 but our practice name is different than the provider name. The provider's name is printed in box 31. Reviewed the 1500 form instructions (where our DMERC carrier instructed us to go for a tutorial) and there was nothing in this that stated where these items are to be placed on the claim form.
I've also been scanning the internet and still can not locate the answer to this.
Can anyone please provide some guidance on this subject.
