We use L3960 (postop shoulder brace) and L3670 (gel pack) for our surgical patients. Medicare stopped paying for L3670 as being included with L3960. BCBS has now processed these claims the same way. Anyone else having this issue, and if so, is there a different gel pack that is reimbursable by insurance?
