Hi, I am billing for orthoris and prothesis company and on the CMS 1500 I read that you need to use "DK" in front of the referring provider name? Is that correct and how do I add that? When I enter the referring provider NPI the name comes up and there is no where to add the DK. Also do you need an ordering provider on the claim as well as a referring provider
