Please can any one confirm Medicare pay for Q4196,Q4101,Q4160,Q4106?

These codes are covered when the conditions outlined in the LCDs are met. The LCD requirements vary depending on which Medicare jurisdiction the provider is billing from. If you search the Medicare Coverage Database for LCDs related to skin substitutes and/or wound care, and find the LCD that applies to your provider, you should be able to locate the specific guidelines.

It is for Texas State. As per below link they say they pay for graft when billed appropriately with CPT.
