Do we need to use modifier SG?

Hello,
We are billing for an ASC and trying to verify certain aspects of ASC billing. Does anyone know if modifier SG is used when billing ASC on a HCFA 1500? This modifier is not needed when billing on a UB. Feedback is most appreciated!

Hi - This modifier seems very payer specific. Medicare Claims Processing Manual, Chap. 26, has this. (https://www.cms.gov/regulations-and-guidance/guidance/manuals/downloads/clm104c26pdf.pdf)

Even so, I've seen some comments that it's best to check the Medicare payer policy.
