I have been really unclear about if the doctor orders "HIV, rpr, cg/cl, gram stain from uretral swab" if I should code that... because the patient only came in to check for std's.

Don't code from orders. A doctor can order anything he/she wants, but that does not mean those labs were drawn that same day. Wait for the EHR entry from the lab tech, and code only those labs.

If you need help matching a lab to a CPT code, your lab tech should have a reference guide from whatever lab service you use to help you with those codes, if the EHR is no help
