One recent topic debated in our office is provider notes vs nurse notes and when each should be used. May I please have someone explain the difference between each and different situations each should be used?

For example: If a patient visits our office for a testosterone injection and the provider tells the clincal assitant the dosage to inject, should the clincical assitant note under their own name with the Doctor as the supervisor, or should the provider note what they had the clincical assistant do?
