I would love to hear how other practices are documenting the topic of sexually active teens. Per a recent reviewe we had with BCBS NC ages 11 and up are to have documented screening for sexual activity and if positive have testing for Chlamydia. Would it be allowed to have this as a standard part of the medical record or should it be under the confidential part of the medical record.
