Can any one tell me what documentation is needed for support of allergen immunotherapy, CPT Code 95165 when results from scratch tests and allergy lab tests were sent to an outside vendor/pharmacy to process the allergen serum injections. Would I also need to document the dosage determined by the physician? Help. Our medical records are being audited and I would like every thing to be documented before submitting.
