Something has popped up in our clinic I am having trouble finding any information on.
The requirement in our practice is to attach the chapter 20 codes onto all accidents and injuries. However, our physicians only want to chart the details on the first visit and for any follow up just want to note that for details refer to the initial chart note. My thought is, if it's not on the chart note I can't add the required dx codes. I have searched high and low and can't find any thing that says you can add the codes if you refer to the past note. If that makes sense. Does anyone have any guidance on where I should look for an answer to this?
