Doe anyone have a clear and concise reference for what needs to be documented in a dictated report for different radiologic tests?

Also, does it have to be a stand alone report or is it ok to just dictate it in the E/M note?

I know there are a lot of changes going on and I am not sure what is still correct or if my understanding was based on the bare minimum or the best case. I haven't done radiology for awhile and I'm not doing it now. The cvt surgeons went to an STS conference and now think they can bill for all the tests they look at even though they weren't actually dictating reports. So they want to know what they have to dictate to bill for them, even though they probably won't get paid.

I would really like to know the answer to this, as well. I am always wondering when I am reviewing something if what they have is enough. I work for a payor and our guidelines simply state "radiology report or equivalent". What's the "equivalent"????????
