If someone calls for an appointment and they are new to the practice....is it required for us to document the conversation IF we have never seen the patient before?
 
If they're just called to make an appointment and nothing more, then there's nothing to document. Payers are only interested in what a provider documents in the medical records about the patient's health conditions, not what an office employee documents about scheduling. If nothing relevant to healthcare was discussed, there's no need to document anything.
 
What about if the patient is sent to another line for triaging? For example - appointments determines the patient may need to be seen sooner than they are authorized to schedule...so they send to a medical assistant for triage. After talking to the patient, hearing the concerns, and appropriating they should be seen sooner.... should this be documented (again on a patient that has not been seen in our office)
