One of my Dr. like to Document his biopsy procedure in his "findings"? Would that pass an Audits? for documentation purpose. is okay for us to code his procedure in the findings? Is it okay if the Dr. documented his procedure any part of his notes? or do they have to be in a certain part of the body of his notes
