Hopefully someone will be able to answer my question. What standard documentation is required for billing/payment for a flu vaccination?

Simple questionnaire.. allergie to eggs? Current ill ? Ever had reaction before ? Then pt signs for consent and usually nurse as witness. Z23 has to be in dx or it will not pay. And admin code G0008 has to be used. There is a new flu code coming out, but won't start paying til Jan 3rd, but sounds like it will retro pay to October. Fun times for billing follow up.
