what is the CARC and the RARC on the EOB? And what is the Blue Cross carrier/plan?

We submit medical documentation with BCBS (Mississippi) on their web portal, myaccessblue.com.

The CARC is CO16. We have received this CARC on claims submitted to BC/BS of Michigan, BCN, Blue Cross Complete, and BC/BS Medicare Advantage plans. We use Availity and we can't find a way to submit supportive documentation for this through the portal. Thank you so much for your help!
