Doc states that patient has cognitive decline. Is there a bullet for this? Thanks for your help.
 
Physical Exam or history?

If you're looking at physical exam bullets ... no. This is too vague a statement to count for any bullet on the 1997 exam.

If you're asking whether it counts in the history ... yes for Psych ROS, -OR- yes for quality (decline) on HPI, -OR- yes for past medical history (but not ALL three for this one phrase).
