Wiki Does CPT 64721 bundle with CPT 25609?

Please help! I got claim denied CPT 64721 denied the payer stated that bundled with CPT 25609 although I added modifier 59
 
I would check payer policy see if they use X-mods. Was it the same arm? Was there a separate incision? Was this incidental finding during ORIF?
