Wiki Does the operative note need signed before it is billed out?

Most of the physicians I work with electronically sign their op notes before it comes to coding for charge entry. We have one ASC that doesn't have that capability yet and a couple docs who "forget" to sign the document even electronically so when we get the dictated op note, it normally goes to the doc to hand sign which, of course, becomes a nightmare to manage. Do we really need to have that note signed and dated prior to coding and billing for that service? What are the rules/penalities for billing a service if the documentation is not signed?

You should not be submitting any claims if medical records are not complete and accurate. Medical record must be signed to be considered complete.
 
I have been telling our providers this, but would appreciate it if you can lead me to a link or where it states this in writing so I can show the providers. Thanks for your help!
