I know this is a dumb question, but there is some confusion at my place of employment.
On an EHR/EMR, how does the DOS need to be documented? Can "Doe, John - (pt #)- John Smith, MD - 3/28/16 3:09PM" , at the top of the progress note, be used as a valid DOS?
