Patient comes in for an annual, Dr. completes annual but also takes care of a problem. documents it all on one document. Can we still charge preventative with an office vist and modifier25? or do we need 2 separate progress notes?

Yes, you can bill a sep E/M visit w/mod. 25- however the HPI,EXAM, and MDM of the problem need to be clearly documented and seperate (meaning clear to read) from the HPI ,EXAM, and MDM of the annual.
