Do you code for both the removal of CRT-D pulse generator and the CRT-P pulse generator? Existing RA, RV & LV leads are reused.

If I'm understanding your scenario right, Id code it this way:

33241 -- Removal of CRTD gen
33221 -- Insert of CRT-P gen, with connection to existing multiple leads
