Wiki Dr. appt. for Child, Only Mom attended what do I bill

Hello out there!!

I have a prescriber who had an appointment with the client (who is a child).
The client did not attend the appointment, but instead the Mother did.
What code should I use? (Originally the appt was to be billed as 90862)
I am thinking code 90846.

I thought I had notes on this topic but I can't find them anymore. I am not 100% sure you would use 90846 because the focus of therapy isn't the family as it was the patient (assuming this since I dont have the chart here), so the intent of 90846 really isn't appropriately met. I will keep looking and if I find something better, I will post

If the appointment was for medication management for the child and now the mother shows up without the child, did the mother just pick up the RX or did the mother have a therapy session about the child? Depending on the situation of the appointment would depend on the code or if it is even code able.
