We've had some instances recently at my office where our pharmacy is dispensing drug for a patient's date of service using both single dose (SDV) and multi dose vials (MDV). These are patients whose payers require drug waste reporting, so we would be reporting with a JZ or JW modifier for the SDV. Has anyone else run into this? How are you billing the drugs? I feel it's most accurate to bill one line for the quantity from the SDV with the JZ modifier (or two lines if there's drug waste to be reported) and a second line for the quantity from the MDV but I just know this is going to cause claim denials.
