We are trying to bill G0434 and the physician visit (example #99231) and almost always we get one knocked out for mutually exclusive. Should we be using modifier #59 on the second line item to get paid for the doctor visit as well? We have an in office lab, hence we are billing for the lab and the doctor visit.
