with the provided info, 380.89 Other disorders of the external ear. You would have to use additional external cause code to identify the cause.
For eg, if there was an associated dermatitis or Pruritic condition or any underlying condition/pathology, like Psoriasis, contact or allergic dermatitis , otitis externa like 380.2x series ( which could be the causative factor), could be assigned along with.
